ECG (electrocardiogram). A single-lead reading taken by touching the crown or bezel for 30 seconds. It can detect atrial fibrillation — an irregular rhythm that raises stroke risk and often has no symptoms. Available on Apple Watch (Series 4 and later), Samsung Galaxy Watch, Fitbit Sense and Charge 6, and Withings ScanWatch. Not available on most budget watches, and regional regulatory approval varies.
Irregular rhythm notifications. Different from an on-demand ECG: the watch checks your rhythm in the background and taps you if it repeatedly looks like AFib. This is the feature that actually catches people, because it works while you are not thinking about it.
Blood oxygen (SpO2). Red and infrared light estimating how saturated your blood is. Useful as an overnight trend for spotting breathing disruption; not a substitute for a medical pulse oximeter. Note that Apple removed and then restored SpO2 on US models through a patent dispute, so check the specific model and region.
Sleep apnea detection. Newer on the scene. Apple Watch and Samsung Galaxy Watch both ship an FDA-cleared feature that looks for the breathing disturbances associated with moderate-to-severe sleep apnea over a 30-day window and prompts you to see a doctor. It does not diagnose; it flags.
Skin temperature. A wrist-temperature baseline built over several nights. Used for illness onset hints, cycle prediction, and sleep quality context. Oura built its reputation on this sensor and still does it best, because a ring sits closer to an artery than a watch does.
Sleep tracking and sleep stages. Movement plus heart rate variability, split into light, deep and REM. Stage accuracy against a lab sleep study is mediocre across every brand; total sleep time is reliable. Treat the nightly score as a trend line, not a grade.
Menstrual cycle tracking. Logging plus retrospective ovulation estimates from overnight skin temperature. Apple, Samsung, Garmin, Fitbit and Oura all offer some version.
Blood pressure. The most over-promised feature in the category. Samsung offers cuff-calibrated blood-pressure readings in supported regions, requiring recalibration against a real cuff roughly monthly. Apple Watch does not measure blood pressure; it flags possible chronic hypertension from 30 days of background data. No wrist device replaces a cuff.
Fitness and Training Features
Built-in GPS. The single most important fitness feature, and the one worth checking carefully. “GPS” on a cheap watch often means connected GPS — it borrows your phone’s location, which means carrying your phone. Built-in GPS works with the phone at home. Dual-band (L1 + L5) GPS, now standard on Apple Watch Ultra, Galaxy Watch Ultra and mid-range Garmin, dramatically improves accuracy in cities and under tree cover.
Workout modes. Preset profiles that change which metrics get recorded. Twenty modes cover almost everyone; the 100-plus counts on premium watches are mostly marketing, though open-water swimming, trail running and strength-training modes are genuinely different from the generic ones.
Steps, distance and floors climbed. The oldest features in wearables, powered by an accelerometer and a barometric altimeter. Floor counting needs that barometer — watches without one silently omit it.
Recovery and readiness scores. Garmin’s Body Battery, Whoop’s Recovery, Oura’s Readiness, Samsung’s Energy Score, Fitbit’s Daily Readiness. All of them blend heart rate variability, resting heart rate, sleep and recent training load into one number telling you how hard to go today. The underlying inputs are more useful than the score.
VO2 max and training load. Estimated cardiorespiratory fitness plus a running tally of training stress. Notifications. Calls, texts, email, app alerts and calendar events mirrored to your wrist, with per-app filtering and a do-not-disturb mode so the watch is not worse than the phone it was meant to replace.
Calls. Bluetooth-tethered calls work on most watches with a speaker and microphone. Standalone calls need an LTE or 5G model with its own cellular plan, typically an extra monthly line fee.
Music. Three tiers: controlling playback on your phone (everything does this), streaming over the watch’s own connection (LTE models), and offline storage of downloaded playlists for phone-free runs (Apple, Samsung, Garmin and most Fitbits).

Safety Features
Fall detection. The accelerometer and gyroscope recognise the signature of a hard fall, sound an alarm, and call emergency services if you do not respond within about a minute.
