Roughly 119.9 million American adults — close to half the adult population — had high blood pressure in 2025, according to the CDC. Hypertension is genuinely silent: no symptoms, no warning, and years of accumulating damage to the heart, blood vessels and kidneys before anything announces itself.
Apple’s hypertension notifications, cleared by the FDA in September 2025 and shipped with watchOS 26, are the company’s attempt to catch some of those people. The feature has been widely misunderstood — it is not a blood pressure monitor, and its detection rate is far lower than most owners assume. Here is exactly what it does, who can use it, and what Apple’s own data says about its accuracy.
What the Feature Actually Does
Your Apple Watch analyses data from its optical heart sensor — the green LEDs on the underside — to observe how your blood vessels respond to each heartbeat. A machine-learning algorithm looks for photoplethysmography (PPG) patterns associated with chronic high blood pressure.
This runs passively over 30-day evaluation periods, starting the day you set it up. If the pattern is consistent with hypertension across that window, you get a notification.
It never produces a number. There is no systolic-over-diastolic reading anywhere in the feature. That distinction matters more than any other point in this guide: hypertension notifications are a screening signal, not a measurement.

The pregnancy restriction is absolute: with an active pregnancy logged in the Health app, the feature cannot be enabled at all. Blood pressure behaves differently in pregnancy, and the algorithm was not validated for it.
How to Turn It On
It does not enable itself, which is the single most common reason people with eligible watches have never received anything.
- On your iPhone, open the Health app.
- Tap your profile icon in the upper corner.
- Under Features, tap Health Checklist.
- Tap Hypertension Notifications.
- Confirm your age and whether you have ever been diagnosed with hypertension, then tap Continue.
- Tap Next through the explanation screens, then Done.
Your 30-day window begins from that moment. Nothing will happen before then — and if you have been wearing the watch for a year expecting a notification without ever completing this setup, the feature has been dormant the whole time.
The Accuracy Question, Answered Honestly
This is where most coverage stops short, and it is the part that actually determines whether the feature helps you.
Apple published a validation paper in September 2025. The algorithm was developed on 3,216 participants, validated on 3,878, and tested on a held-out set of 2,236, compared against the reference standard of average blood pressure readings taken twice daily at home over 30 days.
The headline results:

Read that carefully. Sensitivity of 41.2% means the feature misses close to six out of ten people who have undiagnosed hypertension. Even in stage 2 — the more severe category — it catches only about half.
Specificity of 92.3% is the other side of the same design choice. Apple tuned the algorithm to almost never cry wolf, because a consumer device that generates false alarms at population scale floods primary care with worried, healthy people. So if your watch does notify you, that signal carries real weight.
Screening tests are normally tuned the opposite way — you want high sensitivity so nothing is missed, and you accept false positives that a follow-up test resolves. Apple inverted that. It is a defensible decision for a non-medical consumer device sold to hundreds of millions of people, but it means the feature only ever works in one direction.
A notification is informative. Silence tells you nothing.
What To Do If You Get a Notification
Apple’s flow here is genuinely well designed, and worth following exactly:
- You are prompted to set up a Blood Pressure Log in the Health app.
- Use a third-party upper-arm cuff to take readings for seven days.
- Bring both the notification and those seven days of readings to your doctor.
That is the correct sequence. The notification alone is not a diagnosis; seven days of cuff data is the kind of evidence a clinician can act on immediately, and it turns a vague “my watch said something” appointment into a productive one.
Apple states the feature is not intended to diagnose, treat or aid in the management of hypertension, blood clots, stroke, atrial fibrillation, congestive heart failure or high cholesterol — and, importantly, that Apple Watch cannot detect heart attacks. Chest pain, pressure or tightness means emergency services, not the Health app.
If You Want Actual Numbers, Look at Samsung
Apple is not the only option, and the alternatives take a different approach.
Samsung Galaxy Watch brought blood pressure monitoring to US users on 31 March 2026 through the Samsung Health Monitor app. Unlike Apple’s, it gives you real systolic and diastolic figures alongside heart rate, on Galaxy Watch 4 and newer.
The trade-offs are significant:
- It works from a calibrated estimate, and you must re-calibrate with an upper-arm cuff every 28 days. The cuff is sold separately, so you are buying one either way.
- It is not FDA-cleared. Samsung positions it under the FDA’s general wellness policy for low-risk devices and states plainly that it is not intended to prevent or diagnose high blood pressure.
- Samsung has said passive monitoring showing blood pressure trends over time is coming later in 2026, which would close much of the conceptual gap with Apple’s approach.

What Is Coming Next
Reporting on the Apple Watch Ultra 4 suggests Apple has submitted a real-time blood pressure alert feature for FDA review — a step beyond the passive 30-day pattern detection that shipped in watchOS 26. Supply-chain analysis also points to a substantial expansion in sensor components, which is the sort of change that unlocks health features over several software generations rather than on day one.

The Verdict
Apple Watch hypertension notifications are worth enabling. They cost nothing, run invisibly, and for the minority of cases they do catch, the value is enormous — undiagnosed hypertension found early is one of the highest-leverage interventions in medicine.
But calibrate your expectations to the data. The feature was built to be trusted when it speaks, not to be comprehensive. If you have risk factors — family history, weight, age, poor sleep, high sodium intake — buy a $40 upper-arm cuff and measure properly. A wrist notification is a bonus safety net, not a substitute for the real test.
